If you’d like to skip the paper registration form, registration is just a click away.
We know some folks still prefer pen and paper. Just download the form, print, fill it in and bring it with you.
Because we are a walk-in ER, timing is everything. Please do not submit your registration before you are at the clinic.
Instructions On How To Check-In
- Scan the QR code, then click on the yellow link that pops up
- Click on “First time logging in? Sign up here” button
- Fill out the information to create an account, then click the “next” button
- Enter your home address. You will then need to agree to the terms and conditions
- Click the “sign up” button
- You will then need to verify your account through the email address you entered. Once the email address is verified it will take you back to the check-in website.
- Click the + on the “animals” tab, here you will enter your pet’s information. Please fill this section out as accurately as possible.
- Referring veterinary clinic: is your pets primary care veterinary clinic name
- Altered: is if your pet has been spayed or neutered
- We also need you to select your wishes on resuscitation in the event your pet needs life saving measures. The cost for CPR is $300 – $500
- When you finish filling out your pet’s information you will click on “save changes”
- Next you will need to create an appointment, Click the + on the “upcoming appointments” tab
- Click on the reason for your pets visit at our facility today.
- Euthanasia / end-of-life care: If you have already decided to euthanize your pet and do not need your pet examined by the DVM
- My pet needs to be seen for urgent symptoms: is if your pet is not being transferred to us by another clinic
- Continuing care from day practice: is if your pet has already been seen at another clinic and they are transferring you to our facility for further care and treatment
- Click on the appropriate appointment type and then click select.
- Click on the appropriate level (it will give you examples of each level when you click on them to help guide you to the correct level of urgency.) Once you decide what level to choose, click select
- Answer the question about your pet’s rabies vaccine status, then click select
- Answer the question about your pet’s heartworm prevention and click select
- Enter any medications your pet is currently taking or has taken in the last 72 hours, then click next
- “Submit appointment request” will then popup, choose the correct patient for this appointment. Scroll down to “reason for appointment”, please fully describe why you brought your pet in today, then click submit. No need to adjust the date or time.
- You will then read over and consent via signature to the “emergency consent” form, then click submit
- The check-in process is now completed, and your appointment will show up on our schedule
